routine matters including prescription renewals and appointment
scheduling, please call during our regular business hours.

to the nature of the practice ALL patients must be referred by a physician.
* If
you are a new patient calling to schedule an initial consultation,
please have your insurance card and/or your Workers' Compensation
information readily available.
